Prescribed Medicines in School - the Authorisation to Administer Medication form below must be completed fully and brought to the school office with the medication in a sealed tupperware with the child's name and photograph on the lid.  The dispensing pharmacist's label must be clearly visible stating your child's name, the medication name, the dosage and frequency required.
